Short-Term Stabilization

(45 Day Individual Specialty Care – DISC)
The DISC Program targets patients with severe psychiatric conditions that require temporary intensive support, individualized treatment and integrated care. Over a total of 6 weeks, residents will receive individualized specialty care without having to move from one facility to the next. The Psynergy Spectrum of Care combines distinct levels of care into a single location and eliminates subacute care costs. These services include: Initial Psychiatric Assessment and weekly follow-up, Treatment Plan development and biweekly psychotherapy, a Customized Daily Living Program, Transition Plan development and recommendations for a level of care upon discharge.
